TEEN MAKERSPACE PARTICIPATION AGREEMENT

The Teen Makerspace is intended as a space for teens to express their creativity. Teens are welcome to make the provided craft or work on their own projects with the materials and tools that are provided. Materials and tools will be provided by the REACH and SageFen Maker Fest. Please fill out and sign this participation agreement to acknowledge you understand the expectations and risks.

Conduct Expectations

Respect the rights, safety, and property of others. Listen to staff and volunteers. Respect your surroundings. Notify an adult when leaving the multipurpose room.

Safety Expectations

Listen to staff and volunteers. Use care with sharp tools. Set hot tools down where they belong. Put cap on or retract craft blades when not in use. Use tools for their intended purposes.

Supervision

All teen makerspace activities will take place in the multipurpose and/or patio (weather permitting). The galleries and store are off-limits to unaccompanied minors. Please plan to enjoy those areas with your parents at another time. Minors need to notify an adult if you need to leave the room to use the restroom or water fountain.

Sign In/Out

All participants will sign-in when they arrive and sign-out before they leave.

Assumption of Risk

Participant (or Parent/Guardian, if Participant is a minor) acknowledges and understands that participation in art-related activities may involve certain risks, including but not limited to use of art materials, tools, equipment, and participation in hands-on creative projects. Participant voluntarily assumes all risks associated with such activities.

Release of Liability

Participant (or Parent/Guardian) agrees that the Museum, its officers, employees, volunteers, agents, and representatives shall not be liable for any injury, loss, damage, or harm that occurs during or as a result of participation in the program, except to the extent caused by the Museum’s gross negligence or intentional misconduct.

Engagement & Clean-up

Participation in clean-up is expected as part of this program. Clean-up will begin ~30 minutes prior to the end of the makerspace. If you need to leave early, please clean up your supplies before leaving.

No Transfer or Imputation of Liabiity

Participant (or Parent/Guardian) expressly agrees that responsibility for the Participant's safety, conduct, and actions remains solely with the Participant and/or Parent/Guardian. No liability arising from the Participant’s actions, negligence, or misconduct shall be transferred to, assumed by, or imputed to the Museum.

Indemnification

Participant (or Parent/Guardian) agrees to indemnify, defend, and hold harmless the Museum from any claims, damages, liabilities, or expenses arising from the Participant’s conduct, use of materials, or failure to follow instructions or safety guidelines.

I authorize qualified physicians to provide medical care or treatment deemed necessary in the event of illness or accident during the Teen Makerspace. If I cannot be reached, I authorize local emergency services to transport my child to the nearest medical facility. I also give permission for such first aid as is deemed necessary to be provided on-site or prior to transport.
